EMT Clinicals Experience for Newbies

What Are EMT Clinicals?



Emergency medical technician (EMT) and advanced emergency medical technician (AEMT) students must complete a specified number of hours of EMT clinicals for real-world experience. You work with real patients under the supervision of at least one preceptor. Your preceptor might be a nurse, a paramedic, or another EMT of a level higher than the level for which you are training. Your instructor will give you the information, including contact information, for the facilities with which they have an agreement for EMT clinicals.

If you are at what was previously known as the basic level, you will complete approximately 24 hours of clinicals in the emergency department (ED). At least that is the number required for the state of Iowa. You will focus primarily on patient assessment, because there isn't much else for hands-on experience that a "basic" will get to do. But, take advantage of the opportunity to do real patient assessments. You definitely will need the skill when you begin working.

If you are an AEMT student in Iowa, you must complete 72 hours of clinicals. AEMT students do 16 hours in ambulatory surgery or endoscopy, 24 hours in the ED, and 32 hours of ride time on an ambulance. Generally, you are expected to complete these EMT clinicals in that specific order for a reason. Ambulatory surgery is where you concentrate on getting experience starting IVs. The ED is where you focus on doing patient assessments, but should also take advantage of the opportunity to start IVs, push the drugs that are within your scope, and any other practice you can get. Finally, your EMT clinicals on the ambulance is where it all comes together to prepare you for a career in emergency medical services (EMS).

Surviving EMT Clinicals



Hopefully, you will work alongside dedicated nurses, experienced EMTs, and paramedics who will serve as good preceptors. The reality is, however, that you might encounter ill tempered nurses, medics, and physicians with smelly attitudes who view you as a bother and an interference rather than as a student needing to learn. This can be a real challenge, particularly for the EMT at the basic level doing EMT clinicals for the very first time.

Try to find one or two pleasant nurses or medics with whom you can work and who will be the ones to sign off on your clinicals sheet. You might be advised to just "jump right in" or to just "involve yourself" instead of complaining about the trouble it can be to get hands-on experience. The truth is that that advice only goes so far. There's only so much initiative that a student can or should be taking with patients, because there is a liability issue.

One charge nurse suggested to an EMT student to do patient assessments after the nurse had completed the assessment. This, she explained, would allow her to get in as many assessments as were required. The problem with this is that it causes a sick patient to be bothered twice necessarily. What sense does it make for the nurse to do an assessment on a patient and then the student repeat the same assessment? It's not difficult to evaluate an EMT student's ability to do patient assessments. If he or she does a good job, that student could then do the assessment for the nurse. This allows him or her to get the experiences needed while being a real help to the nurse.

EMT clinicals should be a true learning experience. They should be challenging in the sense of putting the student's book knowledge into practice, not a challenge to even get hands-on experience. If there's a feedback system for reporting on your experience doing clinicals, take the time to give constructive criticism as well as praise. If there's no such system, but your experience was a bad one, try to bring it to someone's attention. If you are an experienced nurse, medic, or doctor, please remember that students need guidance, not an attitude. A huge thanks to the many wonderful nurses, medics, and doctors out there who are being great preceptors to EMT, AEMT, paramedic, nursing, and other students training to be good health care providers.
