EMT Basic Training and EMT experience: Bridging the Gap - Part I
Oxygen by Non-Rebreather
Emergency medical technician (EMT) basic training, at least during the year 2010, always emphasized a need to give every patient supplemental oxygen by non-rebreather at 12 - 15 (liters per minute) lpm. In the real world of EMT experience, however, you'll find that this "mandate" just can't and should not always be followed. Oxygen saturation levels, commonly referred to as "O2 sats," often are in the high 90's on room air in non-smoker patients. Unless local protocols require the giving of supplemental oxygen by non-rebreather for all patients, many EMTs and medics won't give it. Also, patients in respiratory distress and pediatric patients will usually resist a mask being placed over their nose and mouth. EMT basic training stresses the use of the non-rebreather mask, but EMT experience will teach you that use of a nasal cannula is much more practical.
Full Body Immobilization and Pulse, Motor, Sensation Check
EMT basic training stresses the need to check what most medics refer to as "PMS," which stands for "pulse, motor, sensation." When a patient needs to be back boarded (placed on long board/full body splint), PMS should be checked BEFORE he is placed on the board and AFTER he is secured to it. Depending on the emergency medical services (EMS) system in which you work, you might find that the medics are careless when it comes to this check. Pulses should be checked, the patient should be asked if he can move his extremities, and sensation should be checked. This is as simple as checking the pulses, asking the patient to wiggle his toes and fingers, and pushing against the soles of his feet to see if he can push back. You should also ask him to squeeze your hands with his hands. Again, this should be done before and after he is placed on the back board. It isn't always practical or possible to implement every single thing taught in EMT basic training such as the example above with supplemental oxygen. However, too many things, such as PMS check, are being ignored in EMT experience in the real world.
Interventions for the Apneic Patient
EMT basic training should thoroughly cover the use of oropharyngeal and nasopharyngeal airways when ventilating the apneic patient. These devices are often used before upgrading to more advanced airways such as the Combitube® or the King LT®. Many states allow the EMT basic to use these advanced airways. However, it is almost always an EMT paramedic to whom use of the Combitube® or King LT® will be left. Even when the entire attending crew is at the basic level, these advanced airways, might not be used. Local protocols and the "trend" of the area in which you live dictate EMT basic use of these airways which are usually left to the paramedic who is unsuccessful at intubation. If you really want to advance in the practice of emergency medicine, you're highly advised to go on to study paramedicine instead of stopping at the EMT basic training level. However, gaining EMT experience at the basic level while training to be a paramedic is a good idea for some people.
