Checking a Pulse by Machine or Man



Many EMS systems use a pulse oximeter that will display the reading for oxygen saturation and heart rate. While EMT basic training teaches the importance not only of pulse rate, but also of pulse quality, EMT experience will expose you to medics who almost always leave heart rate to the pulse oximeter. However accuarate the sensor in this valuable medical machine might be, it does not tell you the quality of the patient's pulse. The rate might be 60 beats per minute; but, is the pulse strong? Is it weak or thready; is it regular? When time permits, a good medic takes pulse the old fashioned way.

The Taking of Blood Pressure



EMT basic training always stresses the need to take a blood pressure on a bare arm, and there are EMS systems that require, by protocol, that baseline vitals include a manual taking of blood pressure. In real world EMT experience, however, don't be surprised if you your co-workers skip taking a manual blood pressure and you might even see them take an automated one over thin clothing.

The Cincinnati Stroke Test



Possible indicators of a cerebral vascular accident (stroke) include any type of paralysis, hemiparesis (one-sided weakness), headache, and slurred speech. Your local protocols may require you, in such situations, to perform the Cincinnati Stroke Test. This test in EMT basic training, however, might be carried out slightly different in real-life EMT experience. For example, your training will probably tell you to ask the patient to close his eyes and extend his arms straight out in front of him for ten seconds. This allows you to observe if one arms drifts or doesn't move at all. When you begin working on real 911 calls, you may discover that your co-workers don't tell the patient to close his eyes and might even help him extend his arms straight out.

Giving Your Radio Report to the ER



Your EMT basic course, when teaching you how to give a radio medical report, probably included an exact order in which information is to be given. For example, you might be taught that there are eleven parts to a radio medical report:

1. Unit identification and provider level
2. Estimated time of arrival, commonly called "ETA"
3. Patient age and sex
4. Chief complaint
5. Pertinent history of present illness
6. Major illnesses in the past
7. Mental status
8. Baseline vital signs
9. Pertinent findings of the physical exam
10. Interventions performed
11. Patient response to interventions

In the real world, you'll always identify your unit, but you might not identify your provider level. Also, while you would identify your unit first, the rest or your report might not be given in the order listed above.

EMT basic training is little more than preparation to begin learning to practice real emergency medicine through EMT experience. That experience must be gained in the real world as not even the most sophisticated simulation laboratory can take the place of the real thing. EMT experience in a small town where few 911 calls are made will never prepare you like experience in a city where any and every type of injury, sickness, danger, threat, and situation occurs. Also, Christian EMTs and paramedics as well as medics with strong Christian values face unique challenges. Let's learn to pray and support one another as we serve God in this field.
