What Is a Glasgow Coma Scale Score?
Emergency health care providers often calculate what is known as a Glasgow coma scale score when treating patients with head injuries. The score obtained is used to help to determine suspicion of the severity of a brain injury. This determination influences things such as emergency transport decisions. Some patients need to be taken to a specialized trauma center to receive the very best medical care.
How the Glasgow Coma Scale Score is Calculated
There are three parts to the Glasgow coma scale: the test, the patient's response to the test, and the score assigned to the response. The three parts of the test are called: opening of eyes, verbal, and motor. Evaluation of the opening of the eyes is a check to see if a patient opens his eyes spontaneously, when spoken to, in response to painful stimuli, or does not open them at all.
A score is given for each type of response in order to arrive at a Glasgow coma scale score. For example, a patient who opens his eyes spontaneously receives a score of 4. This score is added to the score obtained from evaluation of the speech (verbal) and motor (physical movement) tests. This is best understood by looking at a Glasgow coma scale score table such as what roughly appears below:
A Sample Glasgow Coma Scale Score
A patient who opens his eyes spontaneously, can hold an oriented conversation, and is able to follow instructions, receives a Glasgow coma scale score of 15. This is a normal score. They receive 4 for spontaneous eye opening, 5 for oriented conversation, and 6 for demonstrating the ability to follow instructions such as "squeeze my hand." Mild traumatic brain injury might have occurred if the calculated Glasgow coma scale score is between 13 and 14. A score between 9 and 12 can indicate moderate trauma to the brain. Severe brain injury is suspected with a score of 8 or less.
A Glasgow coma scale score is more detailed than the AVPU scale. Emergency medical technicians and paramedics should use the systems to evaluate levels of consciousness that are indicated in their local protocols.
